If You Can't Attend

You may not cancel or reschedule the appointment. If you determine you cannot attend, please contact the person who scheduled the appointment immediately to discuss rescheduling the appointment.

Failure to attend the appointment may affect your benefits or claim. No shows or cancellations three (3) business days or less in advance of a scheduled appointment WILL result in a substantial charge.
